The specialists told me that it's the weak upper jaw and underdeveloped midface that makes the lower jaw protrude, and they didn't recommend moving it. The problem is also that there's is very little room for movement of the jaws since the underbite isn't more than 3-5 mm at most once the teeth are in the correct position.

I do not want silicone implants, but the HA grafts that Arnett/Gunson use are acceptable. I've had eye bags ever since I can remember due to the weak midface, and I hope that HA grafts and/or midface osteotomy can fix that (I've seen plastic surgeons achieve good results with silicone implants).

The last specialist team I visited wasn't actually that bad (the previous specialists didn't even do a thorough examination). They did acknowledge the problem and explained that doing anything without also fixing the midface wouldn't lead to a good result. The problem is that they don't do midface surgeries at all, and as far as they knew no one else in my country do it either. I suspect the situation is the same in most European countries, and it seems that there are very few specialists in the world that have the training and skill needed to perform Le Fort II/III surgeries and HA grafts.

Where did you get that name from? Most surgeons in Europe have a traditional approach where they're only concerned about the bite and not the overall aesthetics of the face.

Mommaerts in Belgium and Raffaini in Italy might be worth a look. Raffaini's website in particular have some amazing transformations.

Have you considered midfacial implants? Yaremchuk in Boston is a good option for midface augmentation, from what I've seen.
