Cowlypso's Quest - Springs!

Okay, I suppose it's time I start the documentary. I finally figured out how to set my camera to macro and get some (sort of) decent pics. Which means I'm ready to bore the pants off of everybody. :D

After debating for several years about getting braces, I've decided to take the plunge. The thing that pushed me over the edge was actually my friend's wedding. I realized that I really just don't like the way my teeth look. I've always been a little self-conscious about them, but seeing those pics really made me realize how much I didn't like them. Here are a few of the offending pics.

So the search for an ortho was on! I started with some referrals from my dentist. Got in with one guy who I was referred to and then picked another randomly from the phone book. Both of them said pretty much the same stuff, and the only real difference seemed to be the price and location. They were both very nice. So I went with the cheaper one. I think the more expensive one was more expensive simply because they offered "extras." They had about three different contests going when I went in for my consult. Not what I want to pay for.

The decision was made. I'm going with Dr. P. His staff is very nice, I like him, and they seem to be fairly flexible with scheduling. The next steps were records and the perio consult. Dr. P requires a perio consult for all adult patients.

Records exam was certainly interesting. They offered me about 10 flavors to choose from for the impressions. I chose the pina colada. She put in the lower mold, and I thought to myself "huh, tastes vaguely like plastic, and not at all like pina colada..." Just about that time, the assistant realized that she had forgotten to put the flavoring in. So she promised to put it in for the upper impression.

First attempt at the upper impression led to crud sliding down my throat, and the most awful taste! Adding the pina colada flavoring caused the stuff to have this most awful chemical taste with a hint of pineapple. I'll take the plastic-taste any day! And, as it turned out, I took it twice more that very same day. On the first attempt, my incisors scraped the front of the mold and it was no good. Same with the second. But, third time's the charm. The assistant was great, though. For the second two, she changed the way she applied the pressure to the tray so that the stuff oozed more toward the front of my mouth and not down my throat.

Impressions are horribly messy business, especially when you have so many attempts in a row! I ended up with the stuff all over my face, in my hair, and even up my nose!!!

Next stop was the perio. Very nice office (a little too nice, if you know what I mean...). But the staff was friendly and the guy was nice. Best of all, after torturing me with that little metal probe, he determined that my gums are absolutely perfect. No need for any further consult. Green light for braces.

And now we are current. I am eagerly awaiting my final consult with the ortho to see what series of torture devices he has planned, and exactly how long it will take (and whether he'd like my arm, my leg, or both!).

Dr. P's summary of the problem:
-deep overbite
-overjet
-upper molars too far forward in relation to lowers on left side
-crowding of lowers

The preliminary plan? Upper and lower braces (I've chosen full metal for cost). Also, likely extractions. I'm probably going to end up losing 4 premolars. :cry:

Actually, your teeth already look very nice, I hope it is going to be a quick journey for you!
Interesting issue with the molds, now I can see it has an advantage that I had no choice of tastes :lol:

Hmmm....

Both orthos that I saw said it would take 2.5-3 years!!! :shock:

Dr. P wrote down 32 months for the time estimate. Why so long? It seems like I'm going to be wearing them longer that a lot of people who appear to have much more disorganized teeth than me.

I was starting to have doubts here for a minute (if my teeth aren't that bad, is it worth so much time and money?), but I've gotten that under control. Even if they don't look too bad, it will be nice to fix the bite, and I definitely don't like my overjet!

Just contemplating my bite here and looking at the pictures... I never realized that those upper second molars were tilted out like that. I guess that explains why I have trouble fitting a toothbrush back there on the buccal side. Also, I've just realized that on the left side, my second molars don't even meet! And on the right side, the lingual edges of the second molars meet, but that's about it. And I can see that the bite is off on the left side in the pictures, too.

B-day is a split session. January 4 is spacers in the back, brackets in the front. January 11 is molar bands and archwires. So my ticker is set for January 11, but I'll get halfway there a little earlier.

Hi Cowlypso, glad you've started your journey too :thumbsup: . Looking ast your dates, it seems we're going to be b-day twins. My uppers are going on on the 10th Jan. Your teeth seem very straight to me! The treatment estimate times are just that - estimates. My teeth are much more wonky and crowded than yours (yours aren't wonky at all), and my estimate is 2 years. My ortho said the teeth straighten out quite quickly, but it takes time to close the extraction gaps (mine will be the 4 premolars like you, although I've only had the top 2 (+ a wisdom tooth) out so far). Here's hoping that our teeth behave really well, and that are estimates turn out to be on the generous side +\+ . When are your extactions due?

My B-day is odd. Jan 4 is spacers and brackets, Jan 11 is molar bands and archwires...

I have an appointment for extractions Dec 18. I may try to move that up a week or so if I can. Tomorrow I find out which teeth will be joining my wisdoms in the sad little plastic bag... Actually, it's not all that sad. My mom bought this fabric with teeth and fairies on it and made me a little bag. So my wisdom teeth are in a plastic bag, inside the little tooth fairy bag. Well, two of them, at least... The uppers came out really nice. The lowers were carved into oblivion in order to get them out of my jaw.

I'll post again tomorrow when I find out all the exciting details!

Okay, so I had my consult today. Very interesting.

X-rays weren't too bad. Bone around the teeth looks okay. The astute orthodontist managed to discern that I have a root canal and some fillings. They showed my class II relationship on the one side, and my class 1.25 relationship on the other (mostly perfect, but not quite...). The lateral view was interesting. I didn't realize how messed up my front teeth are! My front uppers slant outward and are too far forward. What I didn't realize is that the front lowers also slant outward. I guess this is what happens when you suck on your hands for too long as a kid. :roll:

Casts showed pretty much the same thing. My overjet is officially 7mm.

The biggest problem is that my lower jaw is shorter than my upper. He's pretty sure that, with extractions, we can get to "almost perfect" without surgery. He's anticipating a 1-2 mm overjet at the end. Of course, one would always like to hear them say that there will be no problem getting everything to work out perfectly in the end, but there are always limitations. He doesn't want to jump right in with surgery, though (neither do I!!!), so we'll see how much we can do with the braces. In the end, I can always change my mind if I want results better than we're getting. Image

I did ask if I had the surgery, would that be a way to avoid extractions, but he said no. So really no reason to rush into that.

Plan for now, then, as partially related in the metal mouth forum...

Full metal braces, molar bands, etc. He looked into his crystal ball and saw rubberbands in my future. Also, he said he may end up using some sort of spring that goes from the back uppers to bottom lowers. I would assume that it would be a spring that pushes, rather than pulls, to push the lowers forward or the uppers backward or something.

Silly me has to then ask how he can be so sure that the molars aren't going to just move forward. His answer to that was headgear, 10-12 hours at night. Headgear??? No!!!! Actually, I guess I'm not that opposed to wearing it at night. My main concern is that my idiot cats are going to try to play with it while I'm sleeping!

The other ortho I consulted with had mentioned a Nance, so I brought this up and asked if it would be a viable alternative to headgear. He explained his opinions on the pros and cons of the Nance. He said it's not great as a primary method of keeping the molars back, because it can cause the molars to tip. Or, I'm assuming, the direction that the wires go on the Nance while it's anchoring the molars interacts with the forward forces caused by the braces, and this results in the tipping.

Anyway, he agreed that the Nance would be a good idea to keep them from moving forward, but he also wants to use the head gear to apply backward forces and keep them from tipping. Yikes!

He briefly mentioned the mini-screw implants, but I didn't question him about it. He just said that they were relatively new and didn't really give me any more info. I think I'm going to call and ask him about it, but I get the feeling that he's not used them before, and I don't want to be his guinea pig.

So as of right now, here is the plan... Image

December 12: Extractions! Getting the axe are the left lower 2nd premolar, and the 1st premolars in the other 3 quadrants. I guess it worked out pretty well the way things came together there. Since my bite is off more on the left side, he wanted to be able to pull the lowers around a little more there. So pulling the second premolar will give just a little more room. Plus, the premolar has had a root canal, so the life-long prospects for that tooth aren't as great as the others.

January 2: Six-month cleaning. Hopefully they won't find any cavities! But if they do, they've got that day and the next to fix them before braces.

January 4: B-day, part 1. Spacers in the back, brackets up front. I have confirmed that they will be putting ligs on the brackets to pad them a little. I looked at the colors, and I think I'm going with purple first.

January 11: B-day, part 2. This is the one I'm actually counting down with my ticker. Molar bands are going on, and I'm getting archwires.

January 18: B-day, part 3. Nance goes in.

I'm not sure exactly when in all of this mess I get the headgear. Probably either the 11th or 18th...

I'm working right now on my list of questions that I forgot to ask today... Let me know if I'm missing anything. Image

1. Does the quoted cost include all appliances, retainers, x-rays, molds, etc....?
2. What about costs for replacing brackets that fall off?
3. Will I need to wear the headgear for the whole 2.5-3 years, or is it a more temporary thing?
4. Will I have bands just on my first molars, or my second molars too (no third molars in my mouth...)?

Oh, and the part that makes me certifiably insane??? I've agreed to run a review session Tuesday night, 6 hours after getting my four premolars yanked! Image

Well, I suppose I could have read the paperwork that they gave me, but it still doesn't really answer my questions...

I didn't get a written treatment plan. I got a "payment agreement form" and a "potential risks and limitations" form (commonly known as the "covering our rears" form). Image

The most informative portion of the payment forms states: "Additional charges will be made for broken appointments without 24 hours notice, or excessive cancelled appointments, lost or damaged appliances, broken brackets and bands, and treatment prolonged due to poor cooperation or poor response."

Doesn't really detail exactly what is included in the cost. I'm going to have to call up the lady and talk with her, and get some more stuff in writing...
-Exactly how much is charged for a missed appointment
-What if I get horribly sick and call less than 24 hours in advance?
-What if the bracket just falls off for no reason?
-How much do they charge for broken brackets?
-How often do they end up charging more because of "poor response"?
-Is everything that we talked about at the consult covered (the Nance, headgear, retainers, etc.)?

I bet some of that is just in there so they can charge you if things get a little excessive. Like multiple broken brackets. Or being charged for cancelling less than 24 hours in advance even if you are puking up a lung. But I'd still like to get a lot more in writing than what they have given me so far. I'm just that kind of person. Unfortunately, I've learned my lessons the hard way. Image

Okay.... After two phone calls to the treatment coordinator (you can't expect me to remember all my questions in just one call, can you???), I have some good answers. :crazy:

The fee covers everything needed during treatment, including the headgear and Nance, x-rays, impressions, and retainers (first set).

They only charge for missed/late cancelled appointments if they are excessive. And they give you a warning letter before they start charging.

They only charge for broken brackets if they are excessive, or if it is clear that they just fell off for no reason. They ask what you were doing/eating when it fell off, and look for bends in the wires that shouldn't be there, which could indicate eating something against the rules. But I shouldn't get charged if one just falls off.

She's never seen anybody get charged extra for "poor response."

So it's all just there to cover them. :roll:

I feel better now. Hopefully this good feeling will last through Tuesday, when I get my teeth yanked. That's really the point of no return. So far, all I've put into this is time and less than $200. Once I get those premolars pulled, I don't think I can change my mind!

I agree that it sounds a bit worrying. Make sure you get good answers to your additional questions. Also what do they mean by poor response, it is part of their job to get your teeth to "respond".

Okay... that was fun. Not. Extractions are completed. Image

Went out for lunch with my mom (my last "totally normal" meal for the next 3 years!!!). Then stopped home for a quick brush, and off to the oral surgeon. Walked in, quaking in my boots. Filled out a whole lot of paperwork, then went back.

The surgeon was pretty nice, cracked some jokes, and tried to put me at ease. Then, after they got my (incredibly high!) blood pressure and asked about my health form, they started the good stuff.

Got the all-powerful, absolutely wonderful nitrous oxide. I sucked that stuff in like there was no tomorrow. Ah, the tingling body, the feel of my heart beating in my toes, the numb mouth, the crazy, swirly, far-away feeling. I just tried to relax and close my eyes. Image

Doc came in with the needles to numb me up. He used the gel on my gums first, although I probably wouldn't have cared if he didn't. I was pretty far gone! They were the usual novocaine shots, except for one on each side in the palate. Yeowch! It made my nose hurt and I started coughing. But it went away quickly.

Then I got left to swim in my nitrous while the shots went to work. Image

Doc came in and put in the bite block on the left, then pulled the teeth on the right. I guess he started on the bottom, but I couldn't tell. Lots of tugging, and then it finally came out. Doc, being the joker that he was, said, "you're really attached to these, huh?" Top right was next, no trouble there. Then he switched the bite block to the right, and went for the bottom left. He was thinking the crown might come off, but it held and the whole tooth came out at once. The worst one was the top left. No real pain, but I could feel the stuff sort of pulling away. Yuck!

But, it was over very quickly. The assistant started packing me with gauze and they switched me over to oxygen. After I could (sort of) walk straight, then sent me out to the waiting room. I walked over to my mom and bent over to grab my purse. Unfortunately, they had underestimated my bleeding a little, and leaning over wasn't such a good idea. Image

Suddenly, there's blood dripping all over the place! On my shirt, on their carpet... I put my hand under my mouth to stop it, and there was blood dripping out between my fingers. It wasn't actually flowing, but there were quite a few drops. So my mom went and got some kleenex to help slow it down.

They came out and grabbed me and took me back for new gauze. She said that they were slowing down a lot, and should stop soon. She also told me to try to keep my mouth shut. Ha! I can't even feel my mouth. Plus, it's stuffed full of gauze. So I went back out and headed home with no further incident.

Oh, and the best part? When I walked out into the waiting room and bled all over, I was all giggly (still coming off the gas). And, the waiting room had filled up. So there were about 6 people there to witness my amazing feat. Among them, I noticed one of my students!!! This would be one of the students who will be (probably) attending the review session I'm giving tonight, and to whom I will be administering a final exam to tomorrow. And there I was bleeding all over my shirt and the floor in front of him! Image

Ah, the joy. So now I'm home, relaxing on the couch. About 10 minutes until I can take out the gauze and see if I'm still bleeding. At least I can use the opportunity to try to take some tylenol to head off any pain (I did get a prescription for vidodin if I need it). Also, I'm starting to get some weird sensations back. So my numb nose is now feeling like I need to sneeze. And the roof of my mouth is feeling tickly. Image

The good news is that I got to keep my teeth. So now I've got four teeth (including my $600 crown...) in a little box. They actually had special little boxes just for that purpose. I'll post a picture later. But I should probably clean them up a little first.

Love your sense of humour and the last person you would want to bump into during all of this is a student! Good luck with your review.

I felt a relief after my extractions as I felt that it was the point of no return and I could just get on with things. Now a month since my brace was fitted I am so happy that I've started this.

Hope it all goes well for you. Look after yourself in the coming days.

Well, the student was obviously there for some sort of procedure, or at least an initial consult...

I do think I scarred some of the folks in the waiting room though. What's worse when you're sitting there waiting to see the oral surgeon than to see somebody come out, mouth packed of gauze, uncontrollably dripping blood? Image

Gauze change went okay. Still bleeding a bit on the uppers, so new gauze went in. First, I took two tylenol. That was an interesting proposition... I took the pills one at a time, placed them way in the back of my throat, then attempted to pour water from a bottle far enough back in my mouth so that it didn't all run out... Ah, gotta love the numb lips! Image

Still waiting for the lower lip to come back to life. It's totally numb still, just a few little tingles to hint that it might still be attached.

I've already called in my backup for my review session tonight. My friend who is in my department will be coming to help talk for me. But my friend has a 4.5 year old son, so my mom is staying to take care of him during the review. Image

Review went well. My entire face is back in the land of the living now. I'm really glad I had my friend there to speak for me, because my lack of teeth were not amused at speaking.

Oh man you are bringing back memories of my wisdom teeth extractions. Pudding and vicodin. I do believe those were the only things I consumed post-op as well. A lovely combination if I do say so :lol: I was put under general and apparently was quite hilarious when I was coming off. My mom said she kept asking me questions and I was trying to talk but nothing was coming out. She said the look of confusion and frustration on my face was priceless. I will say I was very glad my mom was there. No matter how old you get, it's amazing how much your mom will jump in to take care of you. I'm sure you experienced the same "mommy love" I did cowlypso.

I forgot another funny thing... According to the very helpful pamphlet provided by the oral surgeon... "For about two days, try to chew food on the side opposite the extraction site." Yup. I'll get right on that. Absolutely. :lol:

No problems so far. Soup and pudding last night. Pudding and oatmeal with a mashed banana for breakfast today. Just need to figure out what I'm going to have for lunch, since I really can't chew at all right now, but I'm going to be away from home from 9:30 to 6 today. I do have access to a microwave, so maybe I'll take soup or oatmeal.

My mouth feels so weird, missing all these teeth. I haven't felt like this since I was 10! :GapToothed:

Oh, and in the car last night with the 4-year-old, a very important question came up... Exactly how much is the tooth fairy going to leave me for these teeth???
