Extruding Lower Second Molars

Has anyone wearing elastics had problems with their lower molars extruding too much while wearing the elastics? What was done?

What is your ortho trying to fix with your lower molars? Which way are your lower molars facing?


My problem is tilted molars they look like this on the bottom / \
elastics were tried and after 2 adjustments I got an expander.

My lower 2nd molars are tilted toward the tongue area.
My upper 2nd molars are tilted toward the cheek area.
Hence they don't meet in the middle.

Ortho is trying to remove overjet with Class II elastics, however its causing the 2nd molars to extrude more.

Rory...where are you when I need you?

I get so frustrated that when he tries to fix one thing, he creates another problem, then we try to fix that and it creates another problem.

Chris; that is exactly my problem too!! I have read other threads with this sameproblem. My upper 2nd molars are facing more to my cheek and my 2nd lower molars are toward my tongue. We tried elastics but it did not work for me :( My ortho told me that this is not the case with everyone else though. Elastics do work but it did not work for this particular problem I have. So now I'm wearing a lower expander to upright my molars so they can meet with myupper molar one day! He's not worried with fixing my top molars right now because that seems like a much easier procedure . I hate to say it but I am miserable about 8weeks wearing the expander,my tongue still gets cut up and is raw now. I attached 2 pictures . First one was when they tried uprighting them with elastics:

Gennel...wow yours are inward much more than mine. And it looks like its your 1st molar not the 2nd.

That expander, I do believe he was going to use it once when my 2nd molar was tipping forward but my 2nd molar, at the time, was "acting up like a crying baby" and it was even too painful to try to remove the current band so we left it alone. Since then I've had a mini screw attached BEHIND the 2nd molar and the tipping forward has improved but NOT the tipping INWARD..the elastic from the upper canine to that molar cause it to tipped lingually.

Did the elastic work AT ALL in uprighting the molar because mine doesn't have to come up that much. What weight elastic did you use?

BUT...tell me...what is the elastic on the inside of your band attached to?
I was thinking he could do just that!

I am missing 1st molar and 2nd bicuspid (implants later). And I have to replace that awful bulky crown on my 1st bicuspid that doesnt fit with my upper teeth any longer.

The side of the molarband that faces my tongue has these metal cleats (hooks) and the upper molar has the buccal tube hook that I attached the other end of the elastic to. My ortho used two different sizes and strength elastic. The first was 1/4" light and then the 3/16" Medium
The 4 molars were in some serious pain for 5-7days. Then it stopped hurting.
From what I see in your pictures the molar does not look so bad at all !
The biggest obstacle in my treatment I think are these tipped molars.

I wonder if they can add those hooks because my molar band on the lingual side is smooth?

How long did he try to upright them with the elastics?

Well, it may not look bad but it feels awful when I bite down because it hits just inside the upper not in the middle as it should. Its like it gets "hooked" when I eat. :?

They don't add those hooks it's just a different type of molarband.
I had elastics about 8-10 weeks I think and when I went in for my 3rd adjustment he said that even the stronger elastic that he had me use did not work one bit.My bite was exactly the same. He kept asking me if I was sure I used them all the time. I am super committed on getting this problem fixed and I did wear them every moment even while I was eating. I only replaced them when I brushed at least 5 times a day.
I know he believes me now because my molars have not moved since Igot the expander. I'm willing to wear this longer than usual but I won't agree to surgery if that's what it comes down to.

Hi,

I used to have such elastics like Gennel had, for example here :

It worked pretty well. Until my ortho thought it worked enough and said I could
stop wearing it. The relapse was bad and I had to redo this. This scenario happened
5 times. Yes, I had to do this 5 times, and everytime when I stopped, the issue got
worse again. I think I just had to wear this elastic longer, and maybe create
some overcorrection. Anyway, this elastic works sometimes very well and fast.
You see improvement in a time span of weeks. The elastic was attached to a button
at the lingual side of the lower molar. Later on, a molar band was placed, also
one with a flat surface, but the ortho welded on it a small hook. Since yours is already
in, welding is not possible and a button can't be placed on it either, so I fear the molar
band has to be removed to make this scenario possible.

Chris,

With the missing teeth on that side of the arch, the anchorage is a bit limited, so you may want to discuss your concerns with your doc, and possibly place a lower lingual arch to increase the anchorage for your Class II elastic.

Dr S, why do you suppose Clo had relapse on this elastic configuration?

As for anchorage, can the mini-screw behind the molar do double duty in any way...uprighting the molar lingually? It did a good job uprighting when it was tipped forward?

What about using the arch wire, bending it to upright it?

Last time we tried removing the band it was incredibly painful, but the tooth pain is gone now.

I would rather crown the tooth than go through with that dreaded lingual arch! Is crowning an option in order to get everything to align or is it tipped too much? I have a large filling in it too.

hello, did you any of you succeed in getting the tipped molars straightened? I have elastics on mine at the moment, same problem, upper inner edges barely touching lower outer edges, so my bite is awful.
Ortho said he can only try elastics, nothing else he can do. I got them a bit more into line, then he stopped elastics and they near enough immediately went back where they had been. I
am back in them again, except he has put them from top 3 brackets to 2 lower brackets, as in a kind of box shape but wider at the top. previously, it was elastic coming from little metal things that he put on inside of teeth across to upper outer brackets, where if i bite, i am biting the elastic.

I am looking all over the internet to see how this is usually fixed, i cant find anyone with the same problem except on here. please post a reply if you have any idea if this works, it feels like the lower molars are down too far into the gum and also lower ones too far back on one side, bite on other side is great, really fed up with it all now.
