I have to get my lower bicupids extracted to prepare for lower jaw surgery (BSSO). How long does it take to close the gaps?

Surgery with or without lower extractions????
So my lower teeth are flared out a bit. My surgeon said that he could do BSSO without lower bicuspid extraction and move the lower jaw forward by 7mm and do genio as well. I am also getting my upper jaw shortened. On the other hand, my ortho wants to extract my lower biscuspid and get the lower teet...

Insurance pre-authorization for the 2nd surgeon.
If i get insurance pre-authorized with one surgeon and later switch surgeons for whatever reason. Would i have to start all over again as far as getting insurance approval for the 2nd surgeon or will the pre authorization follow the 2nd surgeon.

Kaiser seems to be one of the more lenient insurance company to approve jaw surgery. But as someone said earlier, don't expect to buy insurance and be approved for an operation right away. Just wanted to share that I just got off of the phone with my insurance company (BCBS) and have found out that they have approved my surgery.
