Changes, I can take my bands off to eat and brush and I also get clicking/popping with some pain (which I did not have before the surgery). Also, I have some pains in my lower jaw when my pain medicine wears off. I am assuming these things are normal since the bands put a lot of pressure on the bite...

I think you'd need to provide a lot more information to get any sort of accurate estimate. From what I've read, it generally ranges between 3 months and about a year.

I am also from Minnesota but I only consulted with surgeons in the twin cites so I don't know dr. Viozzi. However, I've read some good things about the mayo max facial people.

It means that when the lips are at rest, you cannot see the teeth at all. In other words, the lips fully meet.

I would think the first thing you want to do is look at your health insurance plan to make sure there really is an exclusion and it clearly excludes what you need to have done.
If it is clearly excluded then maybe it is correct that you have to go through your employer.

I have a similar problem, although perhaps more severe (my lips barely come together when straining - 15 mm gap at rest). I had teeth removed and braces as a teenager.
