I think it would help with your anxiety to discuss this with your orthodontist. I'm not sure of the state of your bite pre-treatment, but in general if your bite is off and then made perfect by braces, lips will look better as a byproduct. So I wouldn't worry about it.

See if you can get one of the surgeons to make a computer generated 'after' image for you. Dr. Relle in Los Angeles made one for me. The advancement might look better than how you think it would - especially if they do both jaws and ccw rotation.

My surgery with Dr. Relle in LA is July 7th. I'm getting a 3-piece lefort & ccw bsso (i.e. maxillomandibular advancement) for sleep apnea and an anterior open bite.
