Brackets coming off

I have had four seperate instances where the cementation failed on brackets:

* two bicuspids, as noted; and
* one molar bracket twice (i.e., same one both times).

The molar bracket debonded despite the fact that I was eating soft foods both times it happened. Troublesome.
