i email with my ortho. this has pluses & minuses, of course, but she can then answer when it suits her (and she is prompt!). her first reply usually sets off a new set of questions but we get everything answered in a few rounds -- until a few weeks later!

my cheeks still get hung up on my hooks but it doesn't hurt (braced 12 Feb, '08--took about 8 days to toughen up). my friends are used to me manually unhooking my cheek while talking. my lips get stuck a bit, too. lip balm of some sort helps with that.
