Extra cusps on teeth

My teeth are.... unique. I have some normal variants which involve extra cusps on some of my teeth. I'm wondering if anybody else has had them, and if they have had any impact on braces.

Cusp of Carabelli. This can range from just a pit or groove to a full-out extra cusp. Most often found on the lingual side of the maxillary first molar. May occassionally occur on the maxillary second molar. Most frequent among people of European descent.

My cusps of Carabelli happen to be of the very large variety. I literally have an extra cusp dangling off the lingual side of the tooth. See red arrows in picture. My ortho said that he'd be banding my molars. How the heck is he going to get the band around this? Anybody have large cusps of Carabelli with molar bands?

Talon Cusp (dens evaginatus). Extra cusp on the lingual surface of the anterior teeth. Most often occurs on the maxillary lateral incisors, but also occurs on the central incisors and canines. It's a fairly rare trait (varies among populations, but highest frequency reported in any population is 7%). It arises from the base of the tooth near the gum, and extends toward the incisive surface of the tooth. It gives the teeth more of a T shape in cross-section, rather than just a flat shape.

I have these on both of my maxillary lateral incisors and (possibly) canines. See black arrows in picture. I've read that they can cause problems with bite, because the lower teeth can hit the extra cusps and cause damage. I guess I've never had that problem because of my overjet. I'm wondering what's going to happen when I get that fixed.... In some places I've read that they should be removed (which can be problematic, because the pulp cavity can extend into them, and removal then requires a root canal (and therefore "killing" the tooth). I've also read that you should just leave them alone. I don't think anybody really knows. Anybody out there have any experience with these???

I've never heard of this either. Very unique! I'd say the molar bands will be fitted to accomodate your extra "bits" without too much trouble.

Okay, somebody else at least has to have the cusp of Carabelli. I know I've seen it on a few of you, even, in the pictures. Of course, the only pictures I can find them on are the ones where people have brackets and not bands... Wonder if it's a coincidence, or out of necessity.

Anyway, something like up to 85% of many European populations have them. So check your mouths and get back to me. :D

Really, though... I've been looking at pics of molar bands, and I just don't see how they can possibly fit. They are all just big squares. Are they very flexible? I guess from the way they look, I imagine them being fairly stiff, and any extra spaces being filled up with cement. But maybe those of you who have had molar bands can say if they are bendy or stiff.

Cusps of Carabelli are way common, and post no problem....the band gets the majority of its retention from the gingival portion of the tooth, and if the Carabelli cusp is a whopper, all you need to do is contour the upper portion of the band to fit around it...

The small "cusps" on the back of you laterals are again, no prob....

Sometimes they can be quite large (rare) and interfere with occlusal function. The big ones can have pulp chambers making agressive adjustments to the tooth problematic, but again that is rare. Yours shouldn't pose any problems!

hey dont worry about the cusps I have them as well. mine arent as large as your but they are sharp. I havent had any trouble with mine so far and I am 12 months into treatment and have molar bands.
I just had a thought perhaps my point little cusps are why my cleats havent bothered my so much. I guess my tounge was alread used to the rubbing

Hi,

You made me look! I am pretty sure I have them. Yes, I am european descent (Italian to be specific) Hard to see in the mirror but it looks like they are there. And for sure on the front teeth I can feel them. I just thought it was normal. They may have worn down a bit after all these years of having a bad bite and a partial cross bite in back. And for sure they were never as large as yours.

I don't have molar bands so can't help you there but I can tell you my bite is now perfect so I wouldn't worry about it.

You made me look too. I kinda do as well, but they are little wee ones. My teeth (and gums/palate) have lots of grooves in general. It's gotta be the Greek descent. These extra side cusps are actually more pronounced on my third molars (which is considered rare!). The one on my left wisdom tooth is even quite sharp. I never thought it was different until you mentioned it and this has never caused me any problems.

i have always wondered if there was anyone else who had them. i actually have them in the exact same spots as you do, the carabelli cusp on both of my first molars (my former dentist shaved down the one on my #3 tooth), and the talon cusps on the lingual surface of my anterior teeth, but only one of my canines have it (#6). i have never known anyone to have them. i have never had any problems with them, but i have never had braces though. so i'm not sure how that is going to turn out.

I have the talon cusps on the lateral incisors and my canines, I thought it was normal!!

i have the exact same on my upper lateral incisors, thought it was a double tooth at one stage.
my teeth are quite close to the bottom ones now and not touching these extra bits so seems ok so far.
