Can you please post the name of this specialist? I'm in the DC area and am looking to see what I can do about fixing a crossbite that a) didn't get fixed when I was a child and b) has been exacerbated by the loss of the 4 first bicuspids that got pulled before I went into braces. I'm 47 and I'm starting to have gum loss issues from my bad bite --I'd much rather have orthodontics than gum surgery!

I'd definitely recommend my orthodontist, Dr. Andrew Schwartz of Capitol Orthodontics.

If you are having the onset of periodontal gum disease , then I would suggest you get a deep cleaning/scaling done with a periodontist first. I did the same, just before I started ortho to make sure my gums remain healthy. It usually is covered 80% by dental insurance, so its not expensive(around $180). Dr.Claudio Iwanmoto is the one I'm visiting currently for gum issues. His team is excellent.
For the orthodontic specialist , look for Dr.Liliana Calkins or Dr.Stephan Tisseront. Both offer different approaches to re-opening extraction spaces. For example, one suggested I get a distraction osteogensis for my dento-alveolar region at the site of the missing first bicuspids and expand the bone to accommodate new teeth. Another option is to get coil springs and Forsus appliance to advance lower jaw and expand dental arches( you must have good bone structure to support this!) to regain extraction spaces. Both would require implants post ortho. Treatment time is around 18-20 months. Of course there is also the option of maxillofacial surgery( lefort1 and BSSO), but it has it own complications, not to mention its expensive( around $50000 in DC area) and there is a long recovery time. And surgery addresses the symptoms not the cause, so there is a good chance for relapse. Final choice is yours, so please do considerable research.

Did you ever manage to find anyone to do this? I'm based in London and would love to find someone who could help me, I'm 32 and had x4 extractions and braces aged 13, it's taken me this long to put two and two together to realise what caused the ugly pulled-back mouth I've had ever since. Any advice appreciated!

Would anyone mind sharing pics of their profile/mouth to explain what they mean by the negative effect of extractions please?

I had 4 teeth extracted during my first round of braces at 13. I've never thought about how my adult face is a result if this. I don't think my mouth looks bad, although I have a very weak chin. That might be my genes anyway.

I have seen 3 dentists and choose one that seems to know his stuff in Surrey

Woohoo068- could you tell me how the process is going? How much does it cost?

I'm considering getting my teeth pushed forward in the future. My smile is so different now, it feels like my mouth is so much smaller, my lips thinner, my nose larger. I'm just not as pretty as I used to be before I had extractions and braces. It's so frustrating because everyone I speak to about this thinks I'm crazy- they don't notice the difference. But I do, especially when looking at old pictures of me compared to current ones. I wonder if my orthodontist can do anything about it? It's been a year since I had them off and I haven't had the courage to go and speak to him...

It would be great to get some advice from you guys.

I just started reversal two months ago. I had extractions and started closing Mar 16. started to notice the negative effects in Aug and continued drastically. My ortho closed left side first then started right. This is how I was able to tell just how bad retraction is for your face. I went to 5 other specialist that confirmed that I wasnt crazy and to stop closing spaces immediately. i am using invisalign so I simply stopped changing trays but the changes still continued a bit. My original ortho then agreed to open spaces with invisalign.

It can be reversed but it can me a time sensitive matter as far as how difficult it is to make space. Your teeth are actually just tilted before the bone adjust. Waiting you can still do it but may need bone grafted which from what Im being told sounds worse than it is. Some orthos will tell you they cant do it because of the device that they use so it depends the placement of your teeth. I was turned down by one that uses a certain palette expander because the type of movement that the device makes is not suited for tilted teeth but it would be perfect if the bone was already adjusted. there are different devices and options

All of my estimates have been around the same as the cost to get braces to $6000.00. As of right now I am in the original treatment so I have not had to pay additional. I can tell you that I was advised and I also believe that you not have this done by the ortho that did this to you. There has to be an understanding of the reversal process and more than likely if they did the retraction they dont understand whats being done to the face. I am in the process of transferring to a dentofacial specialist.

Jepit, why did you have extractions in the first place? Overcrowding, overjet, skeletal class II?

Hi, thanks for the reply. I actually got the retractions done because my upper jaw was overcrowded, so my two outer incisors were pushing outwards. I wish I'd never done the orthodontic treatment, it was purely for aesthetic reasons- I actually had a lovely smile, I just wanted it straight! Though, my orthodontist never told me my face would change, if I had known I never would have done it. Oh dear, and now I can't afford to pay for the cost of further treatment. It really makes me sad when I look at old photos of myself and see what I ruined.

I'm sorry to hear your story... the reason that extractions are so common is beyond me... I was SO stupid to just go with what the ortho suggested! Usually, I research everything very well, wen it's something I WANT, but this was just something I HAD to do because my crooked upper teeth and deep bite were wearing out my lower teeth, so I was a bit like, "whatever".... I regretted extracting teeth the moment I walked out of the office (and I didn't really want to do it anyway, was just told it was the only way, so i trusted that). Luckily I only extracted one tooth on the crowded side and, after much (way too late) information gathering, I refused further extractions, so she wont be able to move my teeth back much, just a little, and mainly close the remaining gap by moving my back teeth forward. You see, I became very aware of my teeth after extracting that one healthy, beautiful tooth, and realized I was mainly told bs regarding my treatment... she was not just trying to fix MY problem, but to camouflage my class II discrepancy, which I couldn't care less about since my bite was perfectly fine for it's function: to eat! Also, what took me to accept the extractions was that I was told there was no room in my arch for all the teeth, which was a shameless lie! I realized too late that my teeth were crooked/overlapping because my arches had been compressed due to my bad habit of sleeping on my belly, causing too much force on one side of the face. I'm still pretty mad at her for taking me down this path, full well knowing I was not there for cosmetic reasons (as I repeatedly told her) but more mad at myself for being so trusting in her opinion, because I never am... So now I have one tooth less on one side, but the gap has partly filled by straitening the teeth, and the back ones have also started moving towards the front, quite unintentionally so far, but they are going there! I don't care about ending with a slight midline shift. However, like you, I'm starting to dislike several other things, about which I'm trying to reason with her the best I can without loosing my mind... The most bothersome for me is that she totally ruined my smile, and I'm obsessing about it every day! Despite the crooked teeth I had a nice smile, with strong standing central incisors and canines, but she extruded my lateral incisors to pretty much the same length of the central ones, giving me a flat, boring smile and even causing some small black triangles on one side due to the gums not being able to accommodate so much movement. I want those teeth intruded again and recover MY smile because I totally hate how it looks now, but she's fighting my decision to do it, saying this is how it should be... what????? probably 99% of the people I know do NOT have teeth like this! I was also left with a never-ending sensitivity of ALL my left side teeth after a wisdom tooth extraction... Anyway, just to finish my long rambling, I KNOW that on an aesthetic level I probably could benefit a little from extractions due to my convex facial profile, but that's not my goal, couldn't care less, so I won't sacrifice more healthy teeth for the sake of beauty. Plus, I don't want to regret 2 extractions instead of 1! I have lived perfectly happy with crooked teeth for as long as I can remember (almost 39 now) and liked my smile way better before. Were it not for the pain and wear of my lower teeth I would never have gotten into this, ever.

It's really bad that orthos push peoples teeth back because it also brings the tongue backwards potentially affecting airway and increasing the chance of sleep apnea. I apparently have severe bi protrusion class 1 bite with skeletal class 2. So I'm most likely getting 4 premolars extracted and getting double jaw surgery. I would never get them extracted if I wasn't getting surgery.
