bimax protrusion + extractions gave me a sunken face. Please help
Hello,
This is my story, In high school, I had three consultations for braces, one mentioned I had bimax protrusion and needed extractions. The other two did not. We got braces with one who did not do extractions.
However that comment about bimax protrusion always stayed in my head and it really developed a body dysmorphic disorder within me.
After my braces came off, my mouth was a lot fuller which made things much worse. And the teeth flared out a bit
Years later I returned to the doctor who recommended extractions and was treated by him.
But now My face is sunken in and my cheekbones stick out promimently. I have no upper face support and my face is narrow, my nose and ears look big.
The doctor says that 7-8 mm will not do this and that I just aged.
But I saw it happening as the gaps closed. Then when the class II elastics were applied to fix overbite it got much worse.
Now I deeply regret my extractions and looking back at my photos I see a nice young boy with just a full face, and now I see someone who's aged 10 years in 3.
I have had various consultations with other orthos to rectify the problem. None have said I had bimax protrusion, and they all agree extractions should have been avoided because that was just my face.
Some say that 7 mm can cause the sunken look because the maxilla moves with the front teeth and the front tooth structure supports my face. Others say that it doesn't, maxilla stays in place and the teeth move within the maxilla, however even one ortho who said the sunken look is not cause by the extractions acknowledged the maxilla moves with the front teeth.
I have been very depressed at my new look, have pain in my jaw and have been having sleep apnea.
So far my plan of action is to have a new ortho use damons and coil springs to push everything forward (7-8 mm) and then bring the molars forward to meet with tads. Treatment time is ~ 3 years.
I'm looking for help and support to regain my face, thank you.

Sorry to hear that but I can't see the photos.
Negative side effects arrive often with 4 bicuspids extracted but normally this never arrives if you are treated for bimax protrusion.
Maybe the best option would be to retract your teeth 4 millimeters and not 8. You may ask for an Invisalign treatment because having braces three times is too much for the health of the teeth.

Thank you for your reply.
I am currently undergoing reversing the extractions. I am lucky that my teeth have not suffered any root resorption.
My current ortho is reopening the spaces with ovation c brackets and springs.
I t was hard to find an ortodontist to do this, many will tel you it can only be done by cutting the maxilla. So how can that not effect your appearance when bringing the maxilla forward would play a huge part in supporting your face.
At The moment I don' know whether I will place implants or simply move everything forward 4-5 mm.
My ortho is going to reopen spaces to 7-8 mm and let me decide. He believes had they extracted 2nd premolars rather than first I would have been fine.
I had flared teeth from lack of room, not bimax protrussion.
My teeth cam back basically a full 7 mm.
I will update this thread and post photos, because I think it will help a lot of people.
Since my spaces have started to reopen. My congested nostril has gotten better and I can sleep on my back comfortably.
I find it infuriating that orthos do not inform you and deny that these things happen from extractions.

I cannot figure a way to post photos here.
Do have to host them somewhere?

My extractions have opened almost 1 mm and already my face is not as gaunt looking. Of course I have the brackets on that help, but my cheeks look a little fuller.
Nov 8 is my next appt. and my ortho will begin to widen my arc to it's orginal width.

Good news! If you have the possibility to get at the same time a wide arch and a straight profile it will be the best!acmoc wrote:My extractions have opened almost 1 mm and already my face is not as gaunt looking. Of course I have the brackets on that help, but my cheeks look a little fuller.

About the photos, put the original files on attachment, they will appear on the thread

Protrusion is normal for a lot of ethnicities.
Orthodontists need to recognize this and stop ruining peoples faces.

Yes I totally agree, but not for Caucasians that's why I decided to have my 4 first premolars extracted.acmoc wrote:Protrusion is normal for a lot of ethnicities.

Any updates? How much space has been created thus far and for how long have you been under treatment? Curious because I begin extraction reversals next week...

Please could you update your treatment progress!!! I'm currently reversing my extractions too and would really like to hear your situation at the moment.
