extractions

#1 Post by Brahma »

Hallo @all, :)

Brahma is back again. At the moment I feel a bit clueless and confused and sad about that. So if you don´t mind, I´ll complain about that all a little right here, just to get rid of it at least. :?

I spent the last few weeks by searching, reading and asking questions. Now I don´t know anything.
Up to now I got sure about really doing something, but don´t know what yet. I feel like having to make important decisions about things I know nothing about.
I´m looking for a way I can keep all my teeth without surgery (nobody ever suggested that for me). It would be a nightmare to have them pulled and sometimes later hear someone say "you could have kept them".
The ortho I met wants to pull two upper premolars. He said, if I had kept my wisdomteeth, there would have been created enough space for them and one was even grown before it got extracted. But now as they are away, there will be space left in the back and that could affect the jaw joint some day.
So why can´t this space be used for the premolars? They are that much smaller.
One premolar is out of the line and the other teeth have moved forward. Why can´t they be moved back to the place they have come from?
He showed me a pendulum appliance and said he would not do that, but I don´t understand why but have asked three times at least. The other way would be quite easy, he said.
Well, I don´t care about what is easy. I don´t like to imply anything, but maybe he even earns more money by getting teeth pulled? I really can´t tell how they make their priorities.
Everyone tells something different, so I could visit some more until I find someone who at least would have a try (if it doesn´t work they can still be pulled later, can´t they), but that seems to be difficult, because some others even want to pull four ones. :? They are really fast with that in these fields.
I can understand that there are reasons for it and that in some cases it could be the better choice to pull them, but now as I found out it might be possible to keep all, I would at least have tried to make sure that I am really one of those, because it appears to be a bad damage of my body to me.

Do you think teeth can be moved back with invisalign, too? Or would it be that uncommon to do that at all? Or dangerous, because of the greater distance they would be moved?
If you like you can find pix in my other thread.

Sorry for wailing, but I just had to tell someone.

best wishes,
Brahma

You probably need more consultations.
Upper and lower 1st premolars extracted
Uppers braced 4/6/07 & Lowers braced 4/20/07
ceramic brackets and rectangular arch wires
Est. term: 30-36 months
De-banded: 3/04/09 w/ LBR and U&L Essix

Yes, I think I´ll have some more ones. But I really doubt to find one that does not suggest "the easiest way" at once. :( But I do hope.

Hi Brahma

It IS possible to move top molars backwards after they have come forward over time. I know because I am having it done.

I also have a lot of space at the back on my top jaw, because I had 2 premolars pulled when I was a teenager, and my back teeth moved forwards into the extraction spaces.

So my ortho decided instead of taking more teeth out, he would move my top molars back to where they should be, and then there would be space to move my top front teeth back. To do this, he's using 2 temporary screw implants. Its taken quite a long time, but it's working. He managed to create about 6mm space on one side and 4mm on the other side, and that should be enough.

I can't understand why your ortho isn't prepared to try and move your teeth rather than just go for extractions. There are many ways to acheive molar distalisation - pendulum appliance, headgear, and miniscrews to name just a few. They mostly need patience and perseverance from the patient, but for a well motivated person like you seem to be, it can be done. Of course I'm not an ortho, and there might be a sound clinical reason why distalisation of the molars isn't suitable for you. But it doesn't sound like you've been given a satisfactory explanation.

I'd strongly suggest getting another couple of consultations, and stressing that you would very much prefer to have the molars moved rather than have teeth extracted. Your top arch does look very narrow, and I'm sure that quite a lot of space could be created just with braces alone, or an expander.

Hey there.. when I was first told I need to have 2 teeth extracted, I felt like crying, screaming and so on.. when I got the same opinion from a different doctor, I felt like.. crying, screaming again.. :( ..and then, the same conclusion from another doctor.. well, it didn't hurt at all.. was no fun either, but I'm ok :) I knew braces = the only solution for my problem, but still, I didn't the know whole process would be so "nasty".. fillings replaced, extractions.. but it's for my own good, so.. keep my fingers crossed, hoping for the best! :D
