Reverse premolar extractions- new alternative: replace them with wisdom teeth

Hello everyone,

I am too a "victim" of the extraction of four premolars. They were removed bc of some crowding. I have asked various professionals about what options i have to reverse this and restore my facial aesthetics ( get full face and lips back!) as it used to be before i got teeth pulled.
The two most common things to do:

1.) Open up spaces and insert 4 implants ( expensive& risky)
2.) Bring maxilla forward with surgery

And now, i was offered a third option that i havent even thought of, and i hope someone of you guys can tell me what you think:

3.) Space could be created behind the molars by bringing molars forward, and my wisdom teeth could come out. Then wisdom teeth could be fully aligned. To create that space in the back, teeth in the front can be moved forward to get the fullness back i used to have (lips) . The arch could be widened and the four lost premolars basically be replaced with four wisdom teeth,


What do you think?
My concern is that the wisdom teeth will not make a difference, and that it wont really replace the lost four premolars as if they were never gone. I am worried that by bringing the back molars forward, my cheeks could sink in even more due to bone loss or whatever.
I have seen dramatic change in my face- my cheeks have become soo much flatter, my face longer and skinnier, older, tired. Something, orthodontists dont believe me and also havent warned me about. Therefore, i cant trust my orthodontist when he says, moving molars forward to gain space for the wisdom teeth wont affect my face in any way.
I have the feeling that it can.?????
Please let me know what you think. Can the movement of the back molars affect the face in any way.?

I'm not an ortho, so please take what I say with a grain of salt.

If you had to get extractions previously because you did not have room, having molars that are significantly larger than premolars as a replacement is probably not going to fit into your arch properly. And if bone loss is a concern, a more worrying problem is gum recession.

Bringing everything foward could leave you with a protrusive profile especially when you had crowding to begin with. So you might see some changes to your face, but you probably should be prepared that the changes may not make you look the same as prior to extractions.

Just my two cents.

Orthodontist can be very dull.
How could this not change your face.
I am opening my spaces, my ortho will push everything forward 5 mm and pull the molars forward with tads, and widen my arc

Hi. I am in the same exact situation as you. Meaning 4 extractions. Teeth pulled back. Spaces are almost completely closed so this is all recent. Same changes to my face: chin, cheeks, lips. And I know how hard it is and how it sinks your self esteem so I send you my support. I have seen many orthos; they listed the same options you just did.
And funny, my ortho doesn’t admit any changes done to my face (of course he never warned me of this either) and wants to push all teeth forward too. Leaving space behind.
I am a lawyers so.. this is a lawsuit. And... after lots of info adquired I am opening the spaces for implants. Cause the forward idea I think is riskier, the space at the end will give you double chin I believe or other strange changes.
An expert said it could be an idea to put implants in between the mollars. Or before them so not in the place or premolars. So they could be smaller and avoid the 7 mm pre molar implant that could cause recesion. So.. thats an idea too. Good luck!!!! Sending you strenght

In anything new treatment you decide, SUIT. Specially if you’re from the US cause its easier to win a medical negligence there. Gather evidence. Record conversarions.
Any treatment will mean at least 1.2 to 2.5 years and lots of money.

Go to a psyquiatrist to have evidence or moral damage or whatever you call it there. First negociate with the ortho to give your money back or pay the new treatment if you get a no, suit.
And dont keep seein him he will not change his mind and wont accept the mistake for risk of negligence.
Find someone who treats cases as ours. Its hard to find but find an ortho who believes in your case
