Who here has had surgery for long face syndrome or vertical maxillary excess?

#1 Post by akashigg »

Hey guys, new to this forum. I just had a few questions for those who have received orthognathic surgery to correct long face syndrome, or vertical maxillary excess. My questions are, did you notice a significant difference in your facial height? Also did it improve your jawline? Lastly, was it worth it? Thank you!

I was diagnosed with skeletal open bite, which is another way of saying long face syndrome. I'm still in recovery (almost 4 weeks post-op). You can check my instagram in my signature.

Did you notice a significant difference in your facial height?
It's noticeable. My lower face (from top of lips to bottom of chin) is shorter and more proportionate.

Did it improve your jawline?
Too early to tell for sure because I'm still swollen, but I think yes. My jawline is more horizontal. Once the swelling goes down I think it will be more pronounced.

Lastly, was it worth it?
Again, too early to tell, but I think yes. NOT for the cosmetic reasons that you are worried about -- that is icing on the cake. I am happy that I no longer have lip incompetence. My lips comfortably sit closed when my teeth are closed. And I can breathe much better -- I used to choke in my sleep but don't anymore. And I am looking forward to chewing properly once I am allowed to chew.
Skeletal open bite (long face)
Self-ligating braces installed 3 Nov 2015.
Double jaw surgery 17 Jan 2017.
Upper jaw expanded and moved up and forward 2 mm. Mandible moved forward 6 mm.

I had jaw surgery to correct a bunch of stuff, but I also had long face syndrome and my surgeon reduced some (but not all) of the vertical maxillary excess. You can see the before and after pictures here (2-month mark):

I used to show a LOT of gum, especially when laughing naturally. Those pictures actually don't show just how bad it was because I would control my smile when taking pictures. My maxilla protruded quite a bit and it was very unsightly when laughing.

As you can see in the pictures, I'm showing a LOT less gum. I still show some when I'm laughing really hard, but it's quite acceptable! The maxilla protrusion is completely gone. I honestly don't feel like my facial height is that much shorter. I see a difference, but it's pretty slight to me. I think it's because my lower jaw was also rotated upwards, so maybe it balanced out what was removed from the top jaw.

My jawline is VERY MUCH improved! As for if it was worth it or not, honestly it's hard to tell because I did the surgery for several other reasons (sleep apnea, TMJ, overbite). Don't get me wrong; it was absolutely worth it overall, but I can't say for sure if it would have been worth it JUST for the long face appearance issue.
Procedure: Bilateral maxillary and mandibular osteotomy (Lefort I; single piece) & septoplasty
Date of surgery: Nov 30th, 2017
