Yesterday I received my New Patient Packet from the UNC Dentofacial Program, which will be doing an evaluation of my situation next Monday. (The Dentofacial Program is a collaborative effort between the UNC OMFS and Ortho departments.) The idea is that they'll sit down together and collaboratively decide the best course of treatment for my problems. (I'm seeing my ortho for my first adjustment day after tomorrow; I'll ask him where he fits in to the picture, since I won't be receiving ortho treatment at UNC, even if the surgery will be done there.)
Yikes! In addition to the standard medical history forms, there are about 10 pages of psychological questions; I can only guess that they are doing some study on psychological motivations for seeking dentofacial surgery. Included is a standardized psychiatric diagnostic questionnaire (looks like it's evaluating me for anxiety, depression, etc.), and then a bunch of questions on how I feel about my current smile and face. I get asked to pick out what I think my face and bone structure look like, probably so they can figure out how my impression of how my face looks matches up with reality. All this took me a solid hour to fill out...
