Sounds like you need to go for my consultations. Personally, I did five and picked the one that I felt the most comfortable with. Some of them were very different in regards to surgery suggestions, extractions, etc.

The problem i speak with 2 ortho one of u dont need double jaw only genioplasty. Other say before cepholemtric radiograph u need 4 extraction for that lips. Then i said ithink problem is jaws. After he analyizied my radiograp he said: ''Waov in ur values the patients look worse and we did surgery. But your soft tissue so thich and hiding these ask a surgeon who can analyze soft tissue but i dont think he surgery with you'' When im total ready for surgery and refused by 2 orthos. Did there any one who refused by some orthos ?

You've only met with orthodontists? You should consider meeting with some oral-maxillofacial surgeons.

I would meet with surgeons as well. My ortho works with only one surgeons and she insisted I meet with them jointly so I am comfortable with the entire treatment plan. They were a package deal and I am at ease to know they are both on the same page when it comes to my care. while the ortho is important you also want to make sure the surgeon can carry out the treatment plan as well. The surgery is the invasive bit so you need to trust your surgeon 150%.

I have a similar problem as you. Both my upper and lower teeth are protruding a lot because of crowding. Extractions can fix the problem when it comes to looks. You extract 4 teeth and pull the mouth back. For me this is not an option because my tongue is too big. Maybe sliding genio will help?

I'm looking into wilckadontics now. You can move teeth around and make more space, and this would theoretically allow you to reverse the protrusion. I can't find any case studies of people like us with both upper and lower protrusion, so I don't know how significant the procedure will be on looks. I do think it will give me more tongue space though, so I'm seriously considering it.

Honestly, if you can get insurance to pay for it, go for the surgery. I say that because my case is similar to yours and orthodontics did not fix my deficient lower jaw.

Do you have some news since? When I see your photos, the problem looks coming from the lower jaw which is retruded, more than a bimaxillar protrusion. You may ask about the Herbst appliance, an appliance which put the lower jaw forward. So you won't be treated with any extractions.
