I have had 1 appointment with a dentist who offered Invisalign (top and bottom set). I have a very crowded bottom set, but he thought it might be possible to straighten by widening the bite, and doing IPR. He says he has done about 30 Invisalign cases.

2nd appointment with an orthodontist who said that at least one extraction would be necessary, and she would recommend fixed braces on the bottom teeth. She was happy to provide Invisalign on my top set, but has only done 4 cases and feels the bottom is too complex for invisalign unless the person has a lot of experience with it.

To be honest, I'm not keen on the idea of a) extraction and b) fixed braces.

Orthodontist was keen on lingual (which solves the visibility problem) but I really don't think I can go through the lisping and irritation! :(

Totally confused about what I should do.

Have any of you gone with invisalign knowing that it would be a difficult case? :?:

p.s. There are no higher level invisalign providers where I live...

My case was medium to hard and iv had the 4 extractions. I think most cases with crowding need the room the biggest issue iv had is that the molars which were moving into the spaces tipped, so if u have extractions ask them about the chances of them tipping. I live in north west if u are near i can tell u where iv had mine done :)

I was told I had a difficult case. First, I needed all four wisdom teeth extracted. My lower molars and premolars are all moving back so that my canines (that were sticking forward like fangs) could rotate and move backward. Up top, my premolars are moving outwards to make my arch wider and line up correctly with the lower teeth.

My dentist was able to make two different clinchecks. One for the case of no wisdom teeth extracted, but IPR on a lot of the front bottom teeth. The other was with the wisdom teeth extracted and no IPR. So maybe you can get the impressions made and have the dentist make up the clincheck so you can see how it looks and if it seems reasonable. The clincheck will show how many teeth would need IPR. You can look at it, get some movies of how the teeth will move and think about it for a while before going ahead with the treatment.

Like you I wouldn't like the idea of extractions. I only got the wisdom teeth extracted because only one was in completely and I finally admitted to myself that they contributed to my front teeth being overcrowded. I was also told keeping them could cause decay problems for the molars next to them. So all that and me not wanting to wait any longer, I went for the wisdom teeth extractions and no IPR.
