One of the things I've enjoyed reading on the boards is the number of different treatment modalities employed by docs here in the states and abroad...
It has raised a couple of questions in my mind....First, are most of the surgical expansions being done overseas?
and secondly, I haven't seen anyone mention whether their appliances were placed "directly", or one-by-one, or "indirectly" using a tray and doing a quadrant at a time?
Mine were placed one by one......and then two were taken off and put on again, then my canine's bracket came off twice before I left that appointment, haha...I'd be interested to see in addition to this how many of us have the self-litigating braces

One at a time, I think, but I can't even imagine how quadrant by quadrant could be done.
No extractions, no talk about extractions, which is good because I am fond of ALL my teeth, even the rotated ones.

Spacers, Nov. 1; Braced Nov. 8, 2006; Braces off March 26, 2008, just short of 17 months. Glad it's over but the jury is still out on whether the game was worth the candle.

I think I have an idea about how "indirect" method is done. But what's the difference between "directly" and one-by-one? My ortho put the brackets on my teeth one at a time and they are self-ligating.
