i want to ask about surgically assisted rapid palatal expansion ,,,, my first questin is how is it done ? my second question is how does it affect the the nose and the shape of the face ,,,, please help me out

To perform SARPE, the surgeon puts you under, cuts through your palate, and then cuts through the palatal bone. You get sewed back up and then go home. You are fitted with a jaw expander that cranks your palate outwards, just like would be done on a child.
I imagine that cosmetically, it would expand your mid-face. I wouldn't think it would do much to your nose.

SARPE is a really straightforward procedure, but as in any surgery make sure you have a very competent surgeon who you feel confident in and trust. SARPE is not a minor surgery although it is outpatient and you can most likely expect to go home the same day.
First you have a palette expander placed in the roof of your mouth (if you can have your ortho put it in as close to your surgery date as possible). The incision for a SARPE is not actually in the roof of your mouth, so there is no open wound inside, but they do make an incision right under your top lip along your upper arch. They also make another incision behind your nose and then cut down through your palette to seperate it the bone. During the surgery your oral surgeon would turn the palette expander key several times to create an initial "gap" and begin expanding your upper arch. Between 4-7 days after surgery your will probably be able to start turning the key 1x or 2x a day (your surgeon will let you know when to start) in the expander to gradually widen the gap, and each turn of the key is generally .25 mm.
SARPE was unavoidable for me to widen my upper arch and create a functional bite. I had SARPE 15 days ago, and I have been really happy with the healing so far. 75% of the swelling seems gone, bruising is completely gone, and my cheeks are a little fuller (may still be some swelling) and my lips are somewhat wider. I still have some swelling around the base of my nose that I expect to last for another 4-8 weeks but my nose does not seem to have changed at all. I expanded my palette to create a gap of 6.75 mm bewteen my two front teeth and finished turning the expander key last night. It feels like it has happened very quickly but so far has been totally worth and and I am excited for the results.

I had SARPE three months ago. Initially my nose seemed wider, but that fortunately has gone away. My smile is wider, my chin more pronounced, and cheekbones have emerged, which I think are all positives of this surgery.
