Let the race begin !!!
I went in for my scheduled brace day this morning. I had to wait about 25 mins along with two families with all their kids, discussing English things (such as David Beckham), and explaining to their kids what "to have an attitude" meant. It was explained as being somewhere between being "cheeky" and being "rude".
The assistant prepared my 4 front uppers for the braces, applying the stinky glue thing. Then I had to wait some 10 mins with the lip-guard in, which was not so nice, but bearable. The OD applied the braces with great detail, referring to my x-rays, which I never quite understood. I think I got a .14 string, white in colour, and Mystique ceramics with transparent ligs.

Braces, day 0
When I brushed my teeth after lunch I noticed that the string wasn't that firm, and allows for a bit of slack/movement if I push it gently. The OD said that as the teeth begin to move, they will affect the string, moving (pulling/pushing). She gave me a kit that included among other things a pair of pliers to snip the end of the wire if it retracts when the teeth move.
Is this normal, that the string is somewhat loose? Or should it be dead-tight (like a cheese slicer with metal cords)?
I can feel a slight pressure, but less than I expected to be honest. It's not uncomfortable in any way, so far, but I expect (hope) that the teeth will beging to ache.
My RPE eject date has been postponed until 20 July so it will give enough time for the braces and RPE to work together. But that's fine, it will be summer and I will be pool-side waiting eagerly.
First adjustment set for 28 June.

1st adjustment
Today I had an ok adjustment. I only had to wait 3 minutes, time enough to enjoy the AC to cool down from the walk in the Madrid heat. Before the visit, I had prepared a number of questions, including one about my impacted wisdom tooth, which she said was not urgent but in need of extraction once the treatment was done. My other questions were about:
Length of treatment. Is it possible to reduce treatment time with e.g. more frequent visits, e.g. every 4 instead of 6 weeks?
-My OD's respose was in short, no. According to her, the treatment time is independent from adjustment frequency and dependent on biomechanics only. She did however mention/recommend the new high-speed braces (don't know name). These braces were optimized in moving teeth and could reduce my treatment time from 24 months to 18 months. They are metallics though, so not ideal for me.
Implant
-Since I am missing a premolar, my OD wants to open a gap to insert an implant to balance out my teeth and center my midline. I was concerned about feasibility of moving the surrounding teeth to create the space necessary. My OD said it was definitely possible and showed me an example patient report (which showed a gap, but no finished implant). She needs 7mm for a premolar, and that's about 4 months.

She'll fix that at the end with elastics. No problem according to her.
Anyways, the adjustment went fine. Some pain from the putting in place of the new archwire, but within limits. My teeth felt very tight for a couple fo hours, but are now ok. I have a .017 square wire to rotate my left lateral and align the height of the brackets so that she can tilt my incisors forward as a next step.
On July 20 I get my RPE taken out I will also get spacers for my remaining uppers and on July 24 full braces on uppers.
Overall I am medium happy with progress the first month. I do understand that the first archwire is mainly used to loosen up the teeth for further movement, but still, I would have liked to see some more movement. My incisors did have a small overlap which is now gone. However it seems that in aligning themselves, the incisors have moved to the weak side of my mouth and shifting slightly my midline. My rotated left lateral has indeed moved, down and rotated, but not a whole lot. Or is it just me getting impatient I also got metallic string ligs to support the new stronger wire better. Curry lovers, here we go!

I am soooooo excited in counting down the 18 days that remain to get this bloody thing out of my mouth Lately the RPE has returned to bug me quite a lot, messing up my speech and roughing up my tongue. Yesterday was horrible, I sounded like I had just had the RPE put in. But after some intense salt water rinses, my tongue is back to normal. Sometimes I don't notice it's there, but during I would say the last month I have been conscious of the existence of something in my mouth that shouldn't be there and having a feeling of wanting to rip it out.
But only two weeks to go now and as Gloria Gaynor puts it - I will survive.
I almost feel like I will become naked when taking the RPE out, it will be such an experience to see my teeth again. And during 4 days I will be half-naked until my full uppers are put on. Yippie!

7 week progress update - gaps, movment - 1day before RPE out

Tomorrow at 10:30 my RPE exits my mouth. Wow, what a journey it has been! I can't really say I'm sorry. It will be weird not to have it there anymore. But I'm sure my mouth will quickly get adjusted to its new life away from the "maximum security/isolation cell" to a "minimum security prison with only fences around it"
In the meanwhile, I thought I'd share the progress of my 4 uppers that are in braces. I have seen quite a lot of movement since my last revision 3 weeks ago, when I got a stronger, square wire. Interestingly, I now have gaps between my incisors and laterals, on both sides. I think it comes from my incisors tilting outward.

IT'S GONE !!!
I awoke this morning for my 10:00 appointment with a slight hangover from too many beers last night.
I arrive at a crowded dentist's office due to the imminent vacation period, but only had to wait about 10 mins, enough time to read up on the latest Spanish gossip magazines.
I'm called in by the assistant and she asks me (as a joke) if I really, really want to take it out, as they know I've been struggling with the experience. I just laughed. She began by putting some numbing gel on my gums to reduce the pain, and then she grabbed a pair of plyers and turned towards me.
I thought she pulled out the expander AND my teeth, all in one go. Awful, pain, bleeding gums, cracking cement paste (which sounds like the teeth are being crushed) and the worst part - an awful stench from the remains of 5 months of a fixed apparatus in my mouth.
WOW what a foul smell
The assistant said it is normal, and in my case rather "clean". She explained that I am their patient with the best dental hygiene, and they had seen worse. Regardless, there's always some debris
I rinsed, spat out the blood and parts of the cement. Thet threw away the device directly without letting me look at it, which was probably for my own best interest.
The assistant went on to polish away the remaining cement and then put in spacers in preparation for Tuesday's full brace day (uppers only I think).
Then my OD came in from her busy schedule and looked at the results - beautiful was her word.
Without further ado, here are the pictures (with molar bands):
As you may notice, my gums are quite inflammed, after what the assistant called the "slaughterhouse" of removing a cemented RPE. So they ordered me to rinse with Chlorhexidine mouthwash - the one who severly stained my teeth before - to reduce the swelling and heal my gums before the molar bands and braces scheduled for Tuesday. I will follow their order. And in addition, I am rinsing with salt water to boost the gum healing. 2-3x/day.
The first thing I did when I got home was to brush my teeth to get rid of the foul, foul taste of 5 months in isolation. Then rinse with salt water, then with the special mouthwash, then with fluoride.
The feeling in my mouth is weird though. It's so smooth!!! My bite is different. I can set my lower jaw further forward as my front teeth are less inclined now. But also the contact points of my teeth are all screwed up. The right hand side touch first, and I can't really connect with the left side molars. The braces will adjust that I suppose.
I AM VERY HAPPY WITH THE RESULTS!!!
Now I will treat myself to a delicious curry lunch.

Looking a little naked ey?! It looks as though your teeth have improved so much but as you stated, the bite is all funny. I have had that too, my right side hitting a couple millimeters befor the left, but now it's fine.
Good thing they threw out the RPE before you got a chance to catch another whiff of it.

the removal part seemed a bit .. scary .. :/
i have the same expander as you except instead of it being clear its metal so hopefully when i get my expander removed it won't be very painful
BUT .. i still need a bit more turns before i finish all 30 turns .. then the ortho will leave my expander in for 3-4 months to heal .. then it will be the REMOVING part .. OUCH ...

I've been a rather happy man the last week after extracting the expander from my mouth and returning to a normalizing life, wearing brackets only. I really have been enjoying every little moment of speaking uninterfered, prrrropppperrrlyyy announcing every word and speaking loouuuudddly and profoundly.
Since the expander extraction on Friday, my gums have been really swollen. Frequent salt water rinses have helped tremendously. But I was still quite sore going into my second adjustment this morning, to get full uppers put in place.
As a first, I needed the upper molar bands. The assistant put them on easily, in 10-15 mins. Then the assistant started preparing my teeth for the additional braces (apart from the front 4) with a lip spreader and all. When everything was done, she called in the OD to apply the braces. I had to wait 20 minutes for her to arrive, lying head down in the chair. I kinda went to sleep, so I guess it wasn't a big deal, apart from really dry lips and toungue. In comes the OD, applies the braces meticulously , activates the glue, and tells the assistant to begin by putting on a utility archwire, and that's sort of where the problems began.
Today the OD wanted to give me two archwires at the same time - one for general loosening/aligment of the uppers, and a second wire to raise the front 4 to open my bite from Class II, division 2, to become a Class II, division 1 (opening). To achieve that, I needed a thick, square wire with loads of bends and tweaks. The OD went away and the assistant started inserting the thick wire into the molar band tube. OUCH, OUCH, OUCH The end of the wire kept cutting into my gums as the wire needed to be twirled again and again to enter the bracket because of the various bends. OMG, really painful Finally one bracket in place. Then more hard, hard pushing on my sore, sore teeth and gums to be able to get the bent, stiff wire to enter the brackets. The wire kept wringing itself up into my gums where they meet the cheek/lips and digging itself in there like a broad knife. Finally she concedes that she has mismeasured the wire and had to cut it off. A new one had to come on...........
In comes the OD, she disusses with the assistant, and takes charge herself. This time it went somewhat better because she begins from the middle. But when she gets to the end, the left molar band tube, she cannot get the wire to go in because my molars are too rotaded. The molar band tubes are pointing too much inwards. She says that I will need a molar rotator.
Oh no, here we go again.
They put in a device, similar to a TPA, that I have to wear for 2 months to rotate the molars into their correct position. The device is leaner, less invasive than the expander, but it doesn't go as much into the palate, which means significant speech impediment.
I'm right back where I started with the friggin RPE !!
The the OD puts in the thinner wire, and a second thick/stiff wire to lift the front 4. But now without connecting the lateral "new" brackets. They will have to wait til my molars are in position.
To finish it off, I also get a spring between my canine and only premolar on my right side (left in picture) to open a gap for an implant. When I get home, I check my diagnosis/treatment plan and that states I am missing my second premolar (it was extracted when I was young). And now the spring is opening a gap before the first premolar.
The spring is pushing the wrong teeth !
So I call the office, and they tell me the OD will call me back. After 30 mins she calls saying that "yes, you are right, we put it on the first premolar, but that it doesn't matter because the teeth are the same shape and size and there is no difference". Normally I wouldn't accept this, but I do agree with them on the same size/shape. Further, there is no room to fit a spring between my molar (rotated) and the premolar - it's simply too small at this stage. And I did not feel like another visit to the dentist's office today - I'm quite upset and discouraged by the new speech impediment I have to wear another 2 months. Upset at no one really, because it's nobody's fault. But still, very unhappy. I was on my way to "recover" from wearing braces, even in the romance area I was feeling stronger that ever after a weekend without the expander. But now......

During the past 5 months I’ve seen early progress in the alignment of my uppers, then a period of little visual progress (but things happen inside the gums I have been told, i.e. root movement that you cannot see), and lately progress in tilting my front 4 teeth outwards to help open up the bite. Check out the progress pics here, between months 4 and 5. My teeth are less cooperative when it comes to my rotated molar, despite 7 weeks of TPA rotator. But I sense that the lack of progress there was due to “glueingâ€

Filip's story - DISCOURAGED
I’ve been braced for 6 ½ months now and just had my Xⁿth revision.

What can I say, I'm getting frustrated. Impatient.
The last months I feel that there has been no real progress, especially with regards to the spring on my upper right side (left in pic) to open a gap for my missing premolar. There was some movement and mm progress some 2 weeks ago, but since then the teeth seem to have retracted and closed back up. I’m not sure a spring will do the trick.
The problem with the no-gap is that a large part of my treatment hinges on the expected implant. I cannot really move past the initial stage of alignment until the implant is in place. That means, X months to open the gap (6 months in my estimate) and then 3-4 months of implant skeletal bonding before I can actually get the implant put in place - realistically not until the end of 2008! Then a few months of alignment around the fixed implant, then into stage 2, leveling of the bite – realistically not until Q1 2009. And considering my large open bite on the sides, I would expect AT LEAST 6 months of heavy duty elastics to close the Curve of Spee (open bite). Here we are at the end of 2009 before I would see a light at the end of the tunnel. And then probably 3-4 months of tweaking. So all in all Q1 2010 !!! My treatment plan forecast 24-36 months, right on par with my own estimate.
My primary reason for braces was esthetics (aligned teeth), secondary to correct the open bite on the sides, and third to get an implant for my missing premolar.
So I will say the faux-pas - I’m considering terminating my treatment.
I am running out of steam. I’m discouraged by the lack of progress and the gloomy (long term) outlook with few, if any, chances of “sentence reducedâ€

There was some movement and mm progress some 2 weeks ago, but since then the teeth seem to have retracted and closed back up. I’m not sure a spring will do the trick.
The problem with the no-gap is that a large part of my treatment hinges on the expected implant. I cannot really move past the initial stage of alignment until the implant is in place. That means, X months to open the gap (6 months in my estimate) and then 3-4 months of implant skeletal bonding before I can actually get the implant put in place - realistically not until the end of 2008! Then a few months of alignment around the fixed implant, then into stage 2, leveling of the bite – realistically not until Q1 2009. And considering my large open bite on the sides, I would expect AT LEAST 6 months of heavy duty elastics to close the Curve of Spee (open bite). Here we are at the end of 2009 before I would see a light at the end of the tunnel. And then probably 3-4 months of tweaking. So all in all Q1 2010 !!! My treatment plan forecast 24-36 months, right on par with my own estimate.
My primary reason for braces was esthetics (aligned teeth), secondary to correct the open bite on the sides, and third to get an implant for my missing premolar.
So I will say the faux-pas - I’m considering terminating my treatment.
I am running out of steam. I’m discouraged by the lack of progress and the gloomy (long term) outlook with few, if any, chances of “sentence reducedâ€
