I had a BSSO LeForte and double sliding genio about 5 months ago and am still not completely happy with my profile / lip competence.
The genio helped my lips closer a bit mroe naturally, but theyre still naturally open.
My OS said they can't do another sliding because it's already moved as far as possible, but they could still do an implant.
Does anyone know if implants help lip competence, or is it purely cosmetic ie. just the chin legnth.

Yes, an implant can help with lip incompetence. I've seen your pics and you're right that your chin is still somewhat recessed. You would definitely benefit from chin augmentation.
That being said, if you started off with severe lip incompetence, you may never be completely rid of it. I've heard surgeons say that for people who with f.e. severe open bites, some incompetence may just be something they have to live with even after surgery.
On the other hand it's possible that your maxilla wasn't shortened enough.

It really depends, lip incompetence/mentalis strain is caused by a number of factors, usually the upper and/or lower jaws being too long vertically. However, having a short upper lip can also cause this which can't really be fixed because it's a tissue issue, not bone.
A chin implants can project your chin further out but it probably won't shorten the distance between your lips. Go see some really skilled doctors to figure out what they can do.

I just thought about you a few weeks ago and wondered how you were doing so I'm glad to see a new post from you.
I remember back when you had the surgery you seemed to be concerned about your profile just a few weeks after the procedures. I'm sorry that things aren't as perfect as you would like.
I'd recommend another consult with another well-qualified OS. I'm sure you don't want to do any surgery again, but honestly when things don't go 100% from the first one I would always recommend a chat with another surgeon that is less "attached" to your case.
Lip incompetence is a funny thing. Mine got worse post SARPE/MSDO, and now that I'm fully healed from all that I think it got better, but not nearly to the point where it will hopefully be after my 2nd surgery. There seems to be a lot of "biology" at play with this, so I recommend to do as much research as you can before committing to anything.
