Do they make a polishing kit or something to use at home like the dentist has?

Good question...

I think a waterpik might help, as I hear very good things about it - have not bought one yet though...but I hear people saying even after flossing and brushing, gunk still comes out when they use a waterpik.

I'm not sure if it is marketed in the US, but here we have a Colgate "professional" toothpaste, meant for use once a week. It claims to contain the same ingredients as dentist's polishing paste does, (probably far less of it though), but I love it. Two brushing sessions with it and a Sonicare and my teeth feel brand new and sparkly clean.
During the week I use "Clinomyn" smoker's toothpaste, which is very mildly abrasive and does a great job at keeping my teeth white and clean. It's not like a normal "whitening" toothpaste, you can actually feel the difference in shinyness!
