SARPE Surgery

Now that I am two weeks post op I thought I would consolidate my experience here on the SARPE thread for the next group of SARPIES.
I had a unilateral (one side only) SARPE and 2 micro screws placed. My expander was placed a week before surgery. My ortho offered to put it in the day before, but I am very glad that I had the week to get used to it. Yes, it is a pain in the rear end, but it is not nearly as horrible as I had feared. The expander is attached to the second to last molars in the back with bands. The front bands didn't fit well so they attached the front of the expander with wire that wraps around the expander bar and goes thought the gums and is twists around the tooth.
My surgery was out patient and I was home in 3 hours - the surgery itself took one hour. They gave me some meds to take in the morning so I was relaxed when I got there. I suppose the surgery was fine....I was sleeping so I can't say that I remember much. When I woke up, I remember thinking, 'yea, I am finally done´. I was a little groggy, (like when you wake up from a nap) but I wasn't out of it - I knew where I was and why right away.
I did make a bit of SARPE history. Not only did I only have one side released, I had a complication that I had never read about before. My palate split during the surgery and I had stitches up there. That was probably the most uncomfortable part of this whole thing. The OS said the bone was unusually thick and he couldn't get separation with the saw alone so he went for the chisel. Normally he puts a finger on the palate so he can feel his way along, but with my RPE in there wasn't enough space. He had an assistant eyeball it and that is why the skin perforated. He turned 15 times to open, and then closed it back 17 times.
I was on liquid diet for the first 5 days or so. This was more because my palate was too sore with the stitches than anything my doctor said. I am to be on a no chew diet for 6 weeks. Chewing would be impossible at this point anyway because my teeth don't meet anywhere.
Anyway, the only discomfort I had was in in my tmj (the doctor doesn't know why that hurt as he did the surgery with the mouth closed), and from the split palate . I am took Advil and Tylenol in bumped up doses for the first few days, and after day 5 or so I was taking regular Advil. . I was numb from my lower eyelid to my top lip the first day. The under eye swelling was the most bothersome as it threw my vision out of focus, but that only lasted one day. Peak swelling was between day 3 and 4, after that everything got better. Two weeks out I am still puffy. I used ice for the first 48 hours and then switched to heat.
The surgeon placed an Alar stitch to keep my nostrils in place and another stitch in the lip to help prevent it from thinning out too much. The Alar stitch is still quite uncomfortable at two weeks out. It really pulls when I laugh or smile. If I am caught off guard and laugh at something, it hurts enough to bring tears to my eyes. The doctor said to try and keep the stitch in place as long as I can (IE - no laughing or smiling) so it can do it's job. I have no feeling in my gums, or in the middle of my upper lip.
One week post op I went back to the OS and started turning. I do 4 turns a day - two in the morning and two in the evening. The first turn feels like nothing, the second makes my palate sting and the bridge of my nose feel a bit of pressure. It has been easy to turn the key myself - it is attached to a nice long plastic stick and all I have to do is insert the key in the hole and push back till I see the next hole come into view. So far I have done 32 turns and I have expanded 7.9 mm (or so says the OS who measured it today). My gap is 5.2mm big. One front tooth is sitting much lower than the other - the OS says this is normal and to stop freaking out about it. The problem is that I keep hitting the stupid tooth with my spoon, glass, fork....
I have been given the OK at two weeks to use my WaterPik an sonic toothbrush. I also can start back at the gym with light weights and light cardio. I spent a long weekend out of town....shopping. Food choices are limited, but I did well with quiche, fish, soup, yogurt....etc.
Anyway, that is my SARPE experience....well, the first two weeks of it at any rate. I hope it helps someone in the future as so many people here on Archwired have helped me. It's nice to see this thread revived. Thanks, Delag, for taking the time to write up your experiences. It sounds like you're doing great.
It hadn't occurred to me that it might be unusual to have stitches along the palate! Mine had to be cut open deliberately to remove a bony growth/protrusion where my palate had fused strangely - again something I thought everyone had . For me, cutting open the palate was planned, though, and my RPE wasn't fitted until the op, to stop it getting in the way. I agree that stitches in the roof of the mouth aren't much fun.
It's also interesting to see someone else making 4 turns a day. The last turn of the day could be a bit uncomfortable, I found. Have you got much more expanding to do, do you know? I'm still trying to work out how you only release/expand one side but I never did get to grips with the technical side of all this!
I'm RPE-free now and it's amazing how quickly it goes, and how much you get used to it while it's in there.
I remember when I first joined this forum spending an entire evening reading this SARPE thread and it's good to see it continuing as I found all the info really useful

I knew someone else had turned 4 times a day - I just couldn't remember who. Bothe the OS and the ortho seem comfortable with 4 turns so who am I to argue? The nice thing is we don't have to turn for so many days. I do one turn, and then go take a shower etc before I do the next one - seems to work better for me. Right now I am on hold at 32 turns. I saw my OS assistant yesterday and she told me to hold off till I see my ortho because I look pretty close to done. This is the bad part about having an ortho who works out of other offices. They e mailed him photos but I doubt he will tell me anything over the phone. Good news is he is back in town tomorrow, so I really don't have to wait too long.
My crossbite was almost all on one side, so when they did the surgery they cut down the middle and then over the molars on one side only. They say that I will still get the bit of expansion I need on the other side, although I don't see how I will get anything other than tipped teeth. I have never done well with the 'blind trust' stuff.
I agree that it would be a shame to let this thread die out. Let's get our other new SARPE people over here.....Sidney, Kyhwa, Christianhd...you guys out there?

Delag, My cross bite is barely gone. The ortho said that braces can fix the rest of it. My teeth are very enven and one is lower than the other. Actually I think my right tooth is kind of wandring around and my left one has not budged.
I also don't blame you for not trusting the OS. How is your pallette healing?
I turned exactly 7mm and had an 8mm gap. My gap is almost gone (< 1mm) at the crowns of my teeth with about 3-4 mm at the gumline. I don't have my braces yet. I'll get them on May 15th. I'll get a TPA that day too.
Oh and I was only allowed to turn 1 time a day! I ws really bumed abut that. It took forever.
I still don't have feeling in the gums above the teeth on the right side of my jaw. I have very little in my front four teeth and in the gums above my left teeth.

What do you mean your cross bite is barely gone? Was most of your movement in the front? My ortho said he wanted more expansion in the back. I wonder if they pushed you more open in the front and how they do that. If it makes you feel better my teeth are very uneven - one side is more than 1/2 a tooth longer than the other...does that make sense?
May 15 - that is really soon! How long will you have had your expander in? Remember to wax anything that feels even a little rough when you get the braces on. It is better to prevent a canker sore than try to heal one.

Actually my molars did not have much crossbite. The one molar (#30) was the only one and that was the one the dentist told me to pull. There was nothing wrong with it (stupid dentist) but I think it was bothering me so much because of the my bite. My ortho had me stop expanding because any more expansion and my molars would have been pushed out to far -especially on the right side. The crossbite was only on the left.

I just got back from the ortho. He turned the expander BACK 4 times! He said we may have to open it up some more later. UGH! The thing is that my molars are tipped out quite a bit, so he can't judge how far to turn. I got an archwire so that was a bit of a shock. This one is supposed to level things out. I go back on Wednesday. After being stalled for so long, it feels really great to be moving foward.

Yeah, 4 turns back isn't too bad in the general scheme of things. If you begin to re-turn later it may be a little achy is the only thing Oh well..

I'm not too keen on having 2 healthy teeth pulled, but there's just not enough room in my mouth! (I have 2 teeth basically behind my front 2, and my canines above them. My teeth are messed up )

crazyb, I can't imagine your teeth are worse than mine. I kind of wish the ortho would pull some. They are full of fillings, and ugly, gross, most of them ache! Okay, maybe not MOST of them, but some of them. There are times I wonder what I am doing! How does it feel to be out of that RPE? WHy didn't they brace you when they put the TPA in?
Delag, It looks like you are having losts of weird things happen with your SARPE. It would stink to go backwards. Hopefully you'll be done tirning that thing.

I think I wasn't braced because she wants the extractions doing first but then didn't want toput me through wearing the RPE for another fortnight or so. I'm glad actually, that I can have a few weeks being able to talk properly and eat without pain or hassle

A bit of an annoyance though- I booked my extractions today and I have to pay for them because it is classed as 'cosmetic'. Please- if I had room enough to keep them I would, and if that's how it is classed, then it would follow that the whole thing is cosmetic (surgery, etc) surely, whereas of course it's on the NHS because I can't chew/bite properly! Grrrr, I know a lot of you have to pay for your treatment, and I'm not so annoyed with the cost, as with the simple fact my dentist is classing this as cosmetic....yeah, I'm overreacting, I know

I am glad things are headed in the right direction for you. Sorry about the extractions! The spaces will soon close though and the cost and spaces will be a distant memory.
I have spent $4,000 for baraces, about $2,000 for my SARPE, about $300-$400 for my OS visits. I still have atleast one more surgery (upper Jaw advancement only if I am lucky), and I have to get one implant, which who knows how much (I am expecting between $7,000 and $8,000 total). So I know about how it sucks to have to keep paying people.
Just imagine three years or so from now, we'll both have beautiful, functional smiles!

I'm due to have SARPE surgery in two weeks time, I'm having the expander placed into my mouth next week with surgery a few days after that. I was told I'll have to turn the key once for the next ten days. I was also told that I would have a gap in-between my front two teeth, my question is, how long does this gap stay there for? Is it really noticeable? I don't want to be looking a right idiot with a space in-between my teeth!
Badbite - do you know when your next surgery is going to be yet?
Crazy - I am sorry to hear about the extractions. It will be worth it in the end though.
Luke88 - welcome aboard. Sorry to say that you will have a big 'ol gap for a little while at least. Some people mush up wax and put it in the gap to make it less noticeable. I decided to embrace the gap and have some fun with it. How many times in your life can you look like Ernest Borgnine? I do admit I am getting a little tired of my husband calling me Ernest though Are you already braced? You will probably close up a bit quicker if you are already braced - see below.

Too bad because 30 sounds like a much more impressive # doesn't it? He also said that it is much too soon to know if I'll need the second surgery - drat. I hate not knowing. I am still a bit puimmobileffy, and my upper lip is still bugging me. It isnt painfull, just .
