Question Abou Filling the Large Space From Extractions

Hi Everyone!

So I have had my braces on for almost 2 1/2 months and finally got the go ahead to get 4 extractions. I know my ortho knows what he is doing but it just seems like there will be such LARGE holes to fill when the teeth are removed. I'm just curious... do the front teeth move back or do the back teeth move forward... Something's got to move to fill those holes :shock:

And any advice on eating? The teeth I'm getting removed are the ones next to the "canines", towards the back. Could you eat normally after a couple of days?

Sounds like you're getting your first pre-molars removed. I had the same of mine removed about three or four weeks ago, followed by braces about ten days ago.

I ate a lot of soup, mashed bananas, pudding, yogurt, and ice cream those first couple of days (soft food guides on the main forum page are also useful). I suspect if I hadn't developed dry sockets in two of the extraction sites, I would have graduated to big kid food much more quickly. If memory serves, I was eating regular food (salads, roasted vegetables, fish, chowder, apples!, etc.) four to six days afterward, even with the medicated gauze used to treat the dry sockets. I did try to avoid excessively pointy food and anything that required biting in to it. Also, cutting your food up in to small pieces reduces the amount of work you have to do post-mouth entry. Be prepared for entire meals to get stuck in your crevasses though. :D

I think which teeth get moved depend on how your teeth and jaw are currently positioned and what the final goal is. My ortho told me that he wanted to keep my upper molars from moving forward, but yours will probably be different. I'm sure you could ask about it.

I got the two upper premolars extracted as well. Eating is very weird for a few days! After that you get used to it. It's mainly psychological I think, as you haven't got used to missing those teeth yet. It's funny, becuase I figured that my little premolars didn't do much work anyway, but after they are gone it feels like they were huge! My gaps are still open, but I think both the canines are moved back and the second premolars moved forward, obviosly depending on where is crowded.
My advice: eat slowly!

I can't give advice about extractions. But regarding filling in of the gaps. Unless your ortho intends that your molars stay where they are (in which case you would have something like a Nance appliance to hold them back) then it will be a bit of both. Your front teeth will move back, and your back teeth will move forward, but not as much because they have longer roots and don't move so readily. If you get class II elastics to help with gap closure, they usually go from top canine to bottom 1st or second molar, and this will tend to make the top gaps close from the front (as the front teeth are pulled back) and the lower gaps will mostly close from the back (as the elastics will tend to pull the lower teeth forward).

Thanks so much everyone! I got my wisdom teeth out about 2 years (which was probably one of the reasons I am 32 and now in braces for the 2nd time... never should have waited so long!!) but had little to no problems so I can only hope this will go the same way. The "whole meal" stuck in the holes is not going to be fun. How is anyone supposed to eat out in public?? Lol! Guess I will save some money on going out to dinner for the next 6 months :-)

I just had 4 pre-molars removed 2 weeks ago. Just remember after the extraction no smoking and no straw sucking. You don't want to get dry socket. My mouth was sore only from the novocaine shots. I stayed away from sharp foods like hard tacos. After a few days I would find myself chewing away on something to realize the food the was a gap and I was just gumming it. :lol:

I got my braces put on the following week. It's weird when things like rice get stuck between the wire and the gum in those gaps. I ate some some the other day that had little pieces of round pasta in it. A piece of pasta would get stuck there and fill like it was bouncing around like the ping pong balls in the lottery machine. :)

Well, since bits of food get caught inside the holes that are left behind, you can't really see it unless you manage to angle yourself in front of a mirror, with a second mirror in one hand and a flashlight in the other.

Not that I have any experience with that.

My oral surgeon also gave me one of those curved tip plastic syringes to use with salt water in order to irrigate the holes at one of my follow-up appointments. I'm not sure if this is normal or if my holes were extra-deep for some reason, but I also got one when I had my wisdom teeth extracted years ago. Once your finished with the extraction sockets, they can be used to squirt water at cats for hours of entertainment.

As disgusting as this may sound, I always did get a sense of accomplishment squirting all the food out of the holes from my wisdom teeth- lol! I wondered if the holes would be like from my soon-to-be extractions.

Too bad I don't have any cats!!

Just my own experience but my large gaps were filled almost entirely from the front. Back molars have moved a bit - maybe 2/3mm but extraction gaps were filled in the main from front teeth moving backwards. Having said that, my gaps were like 10mm at least, but my front teeth havent moved back by 10mm. See it in perspective. As your arch rounds, your teeth may move back but its not the raw measurement you think it is, i.e my front two incisors havent gone back in my mouth by 10mm, they have rounded to fit the gap of 7mm, which is something different.

Gaps were my biggest preoccupation but, as i have found, my worries were somewhat exagerated. Keep in perspective!!

I totally get what you are saying Britlaw, thanks!! I actually had a consult the other day with the oral surgeon and he said very much what you have said. He said 80% will be filled in from the front teeth (my teeth protrude out instead of pointing straight down) and then 20% would be from the back teeth moving forward. I have one more ortho appt before getting the surgery and I'm hoping he will say the same thing!

I did find out that the one remaining wisdom tooth I have (I was promised it would never have to come out)... has to come out. So that makes 5 teeth coming out in two weeks. What we do for nice smiles- ugh!!
